Suitable PTW assessment / written examination will be conducted to measure participant
understa nding after the training sessions. JGCS authorization will be issued to PTW Receiver,
Issuer & Approver/Authorizer after the result of written examination supported by JGCS
Nomination Application signed by JGCS Managers. Requirement and importance of the PTW
prior commencing of any work shall be incorporated to HSSE Induction as describe in HSSE&S
Training Program (S - 000 - 1654- 0210V).

6 GUIDANCE ON USING THE PERMIT TO WORK

6.1 APPLICATION OF PTW
The JGCS Permit to Work system uses the approval forms , associated certificate s.

• Applications shall be made by the JGCS Subcontractors nominated PTW Receiver that can
be Subcontractors Manager, Discipline/Task Supervisor or Engineer who will be directly
involved in the work activity and had completed and passed the PTW Competency Test
conducted JGCS JVD HSSE.
• PTW Application shall be submitted within 24 hours (or one day in advance) for approval prior
to commence work.
• The PTW application form shall include all information relevant to the scope of works,
including but not limited to reference to all relevant (direct or indirect) documents, drawings,
