7. What if the problem is not solved after implementing the countermeasures?
1. Review the complete process for completeness. Throughout the whole process you had gained a deeper understanding of the phenomenon, which might lead to a restatement of the 6W2H and eventually to the identification of potential causes that were overlooked in the initial iteration.
2. If the above does not provide the solution, this could be a complex multi-cause problem that need a more advanced tool like P-M (Phenomenon- Mechanism) analysis.

NOTE: UPS via YYA should be enough for eliminating 95-98% of all the recurring process in the shop floor. P-M analysis is a very complex and demanding tool and the effort required to complete it demands clear understanding that the condition of the installation/ equipment is in absolute perfect operationg conditions (all standards applicable including their upgrade after the execution of UPS) before planning to start one.
