Sulbactam is asemisynthetic13-lactamaseinhibitor which, when combinedwith certain antibacterials, extends their activity against bacteria that
are normally resistant due toproductionofinhibitable 13-lactamases.Such acombinationis sulbactam plus ampicillin, which has significantly extended the antibacterial activity of ampicillin in
clinical practice. The poor oralabsorptionofsulbactam has made it necessary toadministerthis
combinationparenterally. However, a double ester
linkage ofsulbactam with ampicillin has been developed (fig. I) to produce a prodrug, sultamicillin,
which is readily absorbed orally and hydrolysed by
enzymes in theintestinal wall, releasing ampicillin
and sulbactam inequimolarproportions.Because
the drugs released, sulbactam and ampicillin, are
the same as those in the parenterally administered
