An individual with a midline deviation between the maxillary and mandibular arches without apical base discrepancy requires simple mechanics to tip teeth in a predetermined direction. However, the presence of a true apical base discrepancy requires translatory mechanics to correct the midlines which is more difficult having limitation to the amount of mesiodistal tooth movement. The clinician must properly assess the mesiodistal and buccolingual axial inclinations of the posterior teeth for a gross differentiation of dental from skeletal problems.
