b) Flooding, defined as large volumes of water occupying all or parts of the roadway, shoulders and adjoining areas; reported without delay and in any event within one (1) hour of discovery;
c) Any other event where the Contractor proposes to request issue by the Employer of an Emergency Work Order; reported without delay and in any event within twenty four (24) hours of discovery.
Emergency incident reports shall include:
d) The precise location, timing and extent of the incident, supported by maps/plans and date/time stamped digital photographs;
e) A brief narrative description of the circumstances.

1.7.3 Monthly reports
The Monthly Report shall be submitted to the Project Manager each month no later than the fifth calendar day of the following month and shall comprise the following components:
a) Narrative Report, describing Services and Works undertaken during the period, problems and issues arising and measures taken to deal with them;
b) Report of Compliance with Management and Operational Service Levels, as a basis for payment of the monthly installment of lump sum, in accordance with the format included in Figure 1. The report shall include all non-compliances detected and the nature and timing of works done, and the consequent performance penalty reduction points generated;
c) Progress Report on all Works to be paid on unit price basis in accordance with the format included Part B of the Specifications.
d) Report on any Testing undertaken in accordance with the Contractor’s Quality Assurance Plan or as required by the Project Manager. Tests shall be undertaken in accordance with the specifications and appropriate standards and the detailed test results shall be submitted to the Project Manager in the respective month.
e) Structural Inspection Reports on inspections undertaken under as required by TCCS 07:2013/TCDBVN. First inspection reports shall be provided within two (2) calendar months of the Start Date.
f) Updated Monthly Work Programme, detailing the location, timing and extent of all works the Contractor proposes to complete during the following month.
1.7.4 Data Collection and updating of Road Administration Databases
The Road Administration currently operates the following data associated with the assets being maintained under this conữact, either electronically or on paper.
(a) traffic count and classification data
(b) accident data
Video logging shall also be undertaken, once per year, in order to provide the Project Manager with a visual record of performance. The precise timing and location is to be agreed with the Project Manager in advance of the surveys.
The Contractor shall supply all information necessary to maintain this data in a condition of accuracy, currency and completeness appropriate to the users for which the data is intended.
The Contractor shall provide hard copies and electronic copies of the information to the Viet Nam Road Administration, and the Project Manager, as required.
The times and updating frequencies shall be:
(a) monthly for traffic count and classification data; on the standard VRA forms on the 5th, 6th and 7th days of every month, for 16 hours from 5:00 am to 9:00 pm on the first two days and for 24 hours continuously on the third day
(b) in the eleventh month of the contract and annually thereafter for video logging.
The contractor will be entirely responsible for the quality, accuracy and completeness of all data.
1.7.5 Handover Report
Immediately prior to the completion of the contract the Contractor shall prepare a Handover Report. The purpose of the Handover Report is to provide a smooth transition to the next contract and ensure that the next contractor is aware of any outstanding issues. The Report will:
(a) Summarise any unresolved issues;
(b) Include the most recent complete set of data on the roads covered by the contract, and
(c) Provide the following details:
• A schedule of outstanding defects and liabilities
• Any unresolved issues, especially those that may impact on the next Contractor
• Details of any sensitive issues
• Any ongoing special monitoring or maintenance needs.
c) Any other event where the Contractor proposes to request issue by the Employer of an Emergency Work Order; reported without delay and in any event within twenty four (24) hours of discovery.

Báo cáo 1.7.3 tháng

a) Nêu báo cáo, mô tả Dịch vụ và Công trình được thực hiện trong khoảng thời gian, các vấn đề và các vấn đề phát sinh và các biện pháp thực hiện để đối phó với họ;

E) Kết cấu báo cáo Thanh tra về thanh tra thực hiện theo như yêu cầu của TCCS 07: 2013 / TCDBVN. Báo cáo kiểm tra đầu tiên được cung cấp trong vòng hai (2) tháng dương lịch của Ngày bắt đầu.
F) Cập nhật Chương trình làm việc hàng tháng, chi tiết về địa điểm, thời gian và mức độ của tất cả các công trình nhà thầu đề xuất để hoàn thành trong tháng sau.

video khai thác gỗ cũng phải được thực hiện, mỗi năm một lần, nhằm cung cấp cho các giám đốc dự án với một hồ sơ trực quan về hiệu suất. Thời gian chính xác và vị trí là sẽ được thoả thuận với Giám đốc dự án trước các cuộc điều tra.
Nhà thầu phải cung cấp tất cả các thông tin cần thiết để duy trì dữ liệu này trong một điều kiện chính xác, tiền tệ và đầy đủ thích hợp cho người sử dụng mà dữ liệu được dự định.
Nhà thầu phải cung cấp bản sao cứng và bản điện tử của các thông tin cho Cục đường bộ Việt Nam, và Giám đốc Dự án, theo yêu cầu.
