SAMPLE STATEMENT OF UNDERSTANDING (AGREEMENT ON ROLES AND RESPONSIBILITIES)

Volunteer Service-learning Agreement and Commitment
University of Utah students agree to the following standards, guidelines, and procedures:
1. Maintain professional behavior and demeanor at all times.
2. Maintain confidentiality of agency clients at all times.
3. Maintain a minimum of two hours per week volunteering in your arranged placement.
4. Maintain regular weekly contact with agency supervisors to receive service project activity assignments and to provide necessary reports/feedback.
5. Arrange alternatives with supervisor in case of schedule conflicts prior to scheduled service time.
6. Immediately notify supervisor and/or course instructor of any concern, problem, or incident that transpires during the service-learning activity.
7. Volunteer a minimum of 30 hours over the semester.
The supervisor agrees to provide direct service opportunities for university students. The supervisor also agrees to complete and submit a written partnership evaluation at the end of the semester.
Failure to adhere to any of these standards, guidelines, and procedures will result in immediate termination from the service-learning project and a failing grade for the course.
