Physical exam and ancillary studies
 Vital signs: the patient’s temperature is 37.9◦C, her heart rate is 95, her blood pressure is
120/69, her respiratory rate is 18, and her room air oxygen saturation is 100%.
 General: the patient is awake, alert, and appears mildly uncomfortable. She is obese.
 Head and neck: the patient’s pupils are equally round and reactive to light. Her cranial
nerves are intact. The patient’s mucous membranes are moist. Her neck is supple with a
midline trachea.
 Cardiovascular: the patient’s heart has regular rate and rhythm, without murmurs, rubs,
or gallops.
 Lungs: her lungs are clear to auscultation bilaterally. There are no wheezes, rales, or
rhonchi.
 Abdomen: the patient has positive bowel sounds. Her abdomen is soft and obese with
diffuse generalized tenderness to palpation, most focally in the epigastrum and right
upper quadrant. The patient has a positive Murphy’s sign, with no rebound tenderness
or guarding.
 Extremities and skin: the patient has no edema, clubbing, or calf tenderness.
 Pertinent labs: the patient’s white blood cell count is 18.3, alkaline phosphatase is 80, AST
363, ALT 339, amylase 1946, and lipase 2386.
