1. Ensure that relevant employees know under what circumstances and how to communicate/report accidents and incidents (including ‘near misses’).2. Ensure that preliminary accident/incident information is reported to the appropriate facility management, by competent persons on a timely basis.3. Establish robust accident and incident investigation protocols, including, but not restricted to, maximum delay before investigation; interviews; physical evidence and root cause identification.4. Ensure that accident and incident investigations are carried out by a Manager of seniority proportional to the MDLZ classification of the event.5. Document investigation results, and keep investigation reports as defined in S&E Procedure Accident/Incident Reporting and Escalation. Assign a priority, responsibility and required completion date to each corrective action.
