3-Strikes DocumentMAPA will provide a sample document for keeping documentation of policy violations. This document, called the 3-Strikes document should be introduced whenever you or your Store Manager feels that a violation of company policy has occurred and is serious enough to warrant consideration of possible employee termination. After an employee receives 3 strikes, you or your Store Manager will have a detailed summary of each infraction which you can then cite as reason for termination. Of course, if there is one major instance of employee misconduct that warrants termination, document as much of the incident as possible, retain any and all evidence and let the employee go.
