6 GUIDANCE ON USING THE PERMIT TO WORK

6.1 APPLICATION OF PTW
The JGCS Permit to Work system uses the approval forms, associated certificates.

• Applications shall be made by the JGCS Subcontractors nominated PTW Receiver that can be Subcontractors Manager, Discipline/Task Supervisor or Engineer who will be directly involved in the work activity and had completed and passed the PTW Competency Test conducted JGCS JVD HSSE.
• PTW Application shall be submitted within 24 hours (or one day in advance) for approval prior to commence work.
• The PTW application form shall include all information relevant to the scope of works, including but not limited to reference to all relevant (direct or indirect) documents, drawings, JGCS approved SWMS & JSA, checklists and necessary surveys to be carried out before work commences.
• The work description should be a clear statement of a planned activity, location, and equipment to be used.
• Broad scopes of work of a general nature are not acceptable, and shall be rejected by the
Permit Issuer.
• JGCS approved Safe Work Method Statement is required for all activities, critical and not critical describing potential hazard and its mitigation.
