There are plenty of possibilities, here are some:1) The whole set is already preset (all the tracks pre-chosen and in order) in some software like Ableton live, so all the "DJ" has to do is hit play at the time he wants to play the next track;2) The whole thing is completely fake and he is not mixing anything (not as rare as you would think) - just playing a precorded set;3) The DJ is using software (Traktor, Ableton, VirtualDJ, Serato, whatever) to play and have already checked beforehand that all tracks synch perfectly with each other. So even if he is somewhat free to chose tracks while playing, he can, just by looking at the waveforms, figure out the time to launch a track without needing to check how the tracks would mix. This, in my opinion, generates crappy results: it's always nice to hear the next track, to be sure you have the one you want and how well it mixes at the selected time.I play with software and controller and always use a headphone. I don't like preparing my sets, I always chose the songs while I'm playing. And I like to have the freedom to create some loops and mix songs in different places, not just intros and outros.The one situation that would be IMPOSSIBLE for a DJ not use headphones:1) Someone mixing tracks with CDJs or turntables. Because that would require beatmatching, and no matter how freakingly good a DJ you are, you can't beatmatch without a headphone. If you have temp synced the two songs and start it at the correct time (for phrasing) then it will come in appropriately and you just need to EQ it and be careful with your levels- you can hear it coming in through the monitors and adjust it that way. A lot of DJs nowadays don't spend time beatmatching by ear and just sync the two tracks together. They can always look at the screen and check the waveforms if they need too. It doesn't mean that they're lazy — they use the freed time on 4+ deck stuff and live remixing.Ableton Live. It's live, not a DJ set. Big part of general public doesn't understand that a person performing electronic music on stage can actually perform as a musician. If you see a 'DJ' behind a keyboard or a pad controller, it's most likely that he actually plays his music.

I DJ myself and truthfully headphones are not needed. All headphones do is let you hear the song that you are getting ready to put into the mix. If you practice your live mix and have preset cue points the headphones are not needed. They are useful for precision though

Analog or vinyl DJs who don't use queue points on their records typically use headphones, even if it is for one ear.
Ableton Live performers often queue up entire sets, and each event or song corresponds with a button lit on a controller such as an Ohm64, therefore do not have to hear the song or sample to know when it begins.
