Do you, as the Subscriber, give consent to collection and a third-party disclosure of your personal information as set forth below in accordance with the Personal Information Protection Act of Korea and other applicable laws?
① Person receiving your personal information:
- K-STEMCELL CO., LTD: person entering into the Agreement on Adult Stem Cell Deposit Service (the “Deposit Agreement”) with the
Subscriber
- Commissioned Seller: person commissioned by the Company for promoting the Company’s adult stem cell deposit service
- Partner hospital: , the hospital which has entered into a business cooperation agreement with the Company
② Use of personal information:
- Company/Commissioned Seller: To introduce products and deposit services of the Company; to advertise promotion event (by visit, mail,

e-mail, phone, SMS, etc.)
- Partner hospital: To provide services such as collecting Subscriber ’s blood or adipose tissue.
③ Contents of personal information to be collected and disclosed:
- Company/Commissioned Seller: Personal information included in the ‘Application for Adult Stem Cell Banking’ filled in by the Subscriber, including, without limitation, the Subscriber’s name, occupation, address, email address, telephone number (including cell phone) and etc.
- Partner hospital: Personal information included in the ‘Application for Adult Stem Cell Banking’ filled in by the Subscriber, including, without limitation, the Subscriber’s name, occupation, address, email address, telephone number (including cell phone) and etc.
④ Period for possession and utilization of personal information received
- Company: Expiration of deposit term or ten years after withdrawal of subscription

- Partner hospital: up to three years from the date of consent to disclosure of information

-Công ty/hạ người bán: Để giới thiệu sản phẩm và tiền đặt cọc các dịch vụ của công ty; để quảng cáo thúc đẩy sự kiện (bởi chuyến thăm, thư,

e-mail, điện thoại, tin nhắn SMS, v.v..)
-Đối tác của bệnh viện: cung cấp các dịch vụ như thu thập của người đăng ký máu hoặc mô mỡ.
③ nội dung của thông tin cá nhân được thu thập và tiết lộ:
-công ty/hạ người bán: Thông tin cá nhân được bao gồm trong 'Ứng dụng cho người lớn Stem Cell ngân hàng' điền vào bởi các thuê bao, bao gồm nhưng không giới hạn, của người đăng ký tên, nghề nghiệp, địa chỉ, địa chỉ email, điện thoại số (bao gồm cả điện thoại di động) và vv
