Tôi sinh ngày 28 Tháng 11 năm 1981 tại thành phố Đà Nẵng, Việt Nam. Địa chỉ hiện tại của tôi là 442 Ông Ích Khiêm, Thành phố Đà Nẵng, Việt Nam. Tôi đang cung cấp bản khai này để hỗ trợ xin visa nhập cư của tôi. 2. Tôi lớn lên tại thành phố Đà Nẵng.
